While waiting to schedule or be seen for your appointment, if your symptoms worsen or change, please dial 911 or call 1 (800) 273-8255, or NAMI 1 (800) 950-6264.
In Brevard, Coastal Psychiatric Urgent Care offers same day telemedicine appointments and walk in mental health urgent care (321) 586-5444.
If things are more severe, Palm Point Behavioral Health at (321) 603-6550 offers inpatient stabilization and intensive outpatient programs.
1. COMPLETE ALL assigned forms on your MYIO account
2. TEXT our office (321-638-6818) when forms are completed and SCHEDULE your initial appointment
THE WEEK before your appointment
3. You'll receive an invitation text to SET UP your Spruce account (used for secure messaging and your appointment)
4. Send on Spruce an IMAGE of:
---- your driver's license (for ID verification)
---- your insurance card (as these numbers often are needed in billing or prescription management)
5. SCHEDULE and COMPLETE your test call on Spruce to avoid tech issues the day of your appointment
6. Follow your email prompt to LOGIN to MYIO and COMPLETE the assigned measures, PHQ-9 and GAD-7 (these need completed the week before your appointment which is why they are not assigned with the initial forms)
You will need to have this account set up IN ADVANCE before scheduling your first your appointment. Our team can help coordinate setting up your account. From your MYIO portal, you will be able to complete necessary forms and paperwork.
Once established with your provider, you will be able to use your MYIO portal to:
** see your upcoming appointments
** see your EHR (medications, lab results, and treatment history)
** pay and view bills and payment history
** complete measures and forms
Prior to your first appointment, on your MYIO account, you will complete:
1. Policies and Procedures Forms
2. Clinical History Form
3. Symptom Screener
4. GAD-7 (assigned 1 week before your appointment)
5. PHQ-9 (assigned 1 week before your appointment)
These forms ask for your past medications tried, along with an extensive review of your history. We appreciate your time in completing this before your first appointment. Having this ready in advance allows your provider to spend more of your initial appointment time discussing those details important to developing a customized plan. It also documents necessary information that helps your provider complete insurance prior authorization forms.... so please complete these forms COMPLETELY!!!
BILLING:
We offer transparent and integrated view of your billing through your MYIO app. On MYIO you can directly change your payment method, credit card, and update your insurance information. It also provides you the ability to directly see any current balances, past invoices, and history of payment.
While we bill your insurance as a courtesy, we ALWAYS advise that you check with your individual insurance plan and company for your individual benefits to determine your expected cost and it is your responsibility to be aware of what your insurance covers.
We recommend taking the time to do this as some specific insurance plans have high deductibles or do not have mental health benefits. To assist you in determining your cost - if you are in a plan with a deductible or co-insurance, please reach out and we are happy to provide the commonly used codes so you can check with your insurance to have an estimate of the cost for your visits. Keep in mind, the exact codes used at your appointment may differ depending on the length and complexity of your appointment.
Spruce is how you will communicate securely with our office. It is also what our providers use to connect with you for your telemedicine appointments. Spruce is a secure communication portal that allows secure and direct communication with our office.
You will need to have your Spruce account set up in advance of your appointment.
INVITE:
* A week IN ADVANCE of your first appointment, our team will send you a link inviting you to join SpruceHealth. When you receive your invite, please download the app promptly, and make your account. Spruce can also be accessed from a website.
* Use the + symbol on Spruce to send us a picture of your Driver's License or ID card, and the front and back of your Insurance card
TIP: Enabling notifications from Spruce helps you receive messages from us, though we do try to send manual text messages as well.
TEST CALL:
**Our team will coordinate a test call to make sure all the settings are working. This usually takes less than 2 minutes. As much as possible, the test call allows early identification of any technological issues so your appointment time with your provider can be spent with them, rather than solving tech issues!
Day of YOUR Appointment:
** Log in to Spruce at least 5 minutes before your appointment.
*** See information Below under Checklist for Your FIRST APPOINTMENT
We request for our patients to provide a Release of Information to be able to obtain previous records (lab work, relevant history and diagnosis, previous medication list, etc.) from your previous provider.
*** If you have relevant labs or prior records to include before your appointment, you may use the HIPAA release of information in your Intake Forms to note the previous provider and message our team so we can send it to your previous/other providers in advance to your appointment.
*** If you have copies, you may fax them to us or submit them through Spruce once you have your account set up for secure messaging.
For all telemedicine appointments, it is necessary that you have a working electronic device with a camera and a microphone (cellphone, tablet, computer, etc) and a good internet connection and that you are reachable at your appointment.
ON THE DAY OF YOUR APPOINTMENT
1. LOGIN to your Spruce Account 5 minutes before your appointment
** Make sure you are in a confidential setting that is private and secure for your appointment. Your initial consultation appointment is usually 45 to 60 minutes and your provider will review your information provided, ask additional questions, discuss diagnosis, and formulate your treatment plan
** TIPS: Make sure you are logged in to a strong WIFI connection since Spruce DOES NOT work well if you are moving around, driving, or attempting to use your data
*** You may send a check in message to let your provider and our team know your logged in. When your provider dials you, it will give you an option to Accept/Decline the call. If the initial call does not connect, your provider will send you a message letting you know they tried to call.
While our providers are not double booked - they are booked back to back, so please make sure you are available at the time of your appointment.
For minors, a parent or guardian MUST be present AT ALL appointments to provide information and give consent for treatment. Should you and your child be physically in two different locations, GIVE US 24 hrs advance notice.
Spruce does not offer group calls, so to accommodate these situations, we offer the option to do a group call through WEBEX - which we pay for the secure HIPAA compliant version.
*** You will still need SPRUCE. Even if you use Webex for every appointment, you will still use Spruce for all secure communication and for any appointments you are physically in the same location as your child. Any calls where you will NOT be physically in the same location as your child will be done through Webex.****
This does require coordination in advance, so let us know if you need to use this option.
******** Should the child or the parent or guardian NOT be present at the appointment time for a minor, the appointment will be marked as a late cancel/no show appointment as we cannot complete it without BOTH individuals present. ******
The appointment time is the time set aside to provide information, formulate diagnosis, and discuss treatment with your child's provider. At certain ages and in certain situations, the provider may meet with both individuals and then spend a brief time individually, or the parent/guardian may connect first with the provider and then have the child/minor join.

At Mindful Psychiatry, your first appointment with the provider serves as an initial consultation. During this consultation, the provider will review information completed. The discussion with the patient regarding how their symptoms impact them provides necessary information for conducting a psychiatric evaluation. The intake form is helpful to the provider and serves as a starting point for providers as they speak with the patient. In this time, the provider asks questions and reviews the completed intake forms; revising and adding certain diagnostic questions based on the patient’s verbal responses to elaborate on what is written and reviewed on the intake form. Through this conversation and time spent further exploring the brief responses and/or checkboxes completed on the intake form, the provider is able to perform their job and follow standards of care in psychiatric practice to be able to make medical decisions.
Initial Psychiatric Evaluation Intake Appointments are booked for 60 minutes- typically 45-60 minutes with the provider and 15 minutes or more for the provider to review the chart, complete paperwork, send prescriptions, etc. As this represents a significant amount of time in the provider's schedule, we are unable to reschedule you at our office if you do not have everything completed in advance of this appointment, if you no show, or if you are unreachable at your scheduled appointment time.
The typical wait time to be scheduled for your first appointment may be 6-10 weeks unless there is a cancellation.
