Thank you for your interest in scheduling an appointment with Mindful Psychiatry.
Our office is open Tuesdays, Wednesdays, and Thursdays.
Fridays, Roy Burkhalter offers additional scheduling availability.
At times, our processing of new patients is slower, as our obligation remains first and foremost to our current patients. We do not double book appointments and our providers take a limited number of new patients to maintain availability for their current patients and provide the full attention at this initial appointment. The requested information allows us to ensure if we are the right fit for your needs.
Theresa Lupe, APRN see patients 18 years and older and schedules Tues, Weds, Thurs.
Roy Burkhalter is accepting patients 16 and older and schedules appointment Tues, Weds, Thurs, &Fri.
Nicole Schultz, APRN is currently closed to new patients.
The New Patient Request Form allows us to tailor our provider recommendation and help us offer a good fit. After you complete the New Patient Request Form below, one of our staff will reach out.
Please allow approximately 1-2 weeks for a response. Make sure you have completed your form in its entirety and text us once it is completed. Incomplete information will delay our ability to process your request. If you have not heard back in 1-2 weeks time, we always welcome a second follow up message as certain times of year our responses may be more delayed depending on staffing and provider availability.
NEW PATIENT REQUEST FORM
The form below will lead you through a number of questions about your demographics, current symptoms, medications, and history of treatment.
The Initial Request Form is done through MYIO, which is the HIPAA compliant and encrypted portal companion to our Electronic Health Portal (EHR), Valant. Have your insurance card available when completing. IF you are self pay - type " SELF PAY " under insurance information.
You may click the link below and complete it from your cellphone, tablet, or computer without having to download, print, or scan the document.

Don't Wait!!!
Depending on your selected provider and their current availability, it may take 1 to 3 months to be seen for your first appointment from our initial conversation.
If your symptoms are severe, please DO NOT WAIT for an appointment at Mindful Psychiatry. We DO NOT offer acute treatment services and your mental health is too important to wait. In Brevard County, Coastal Psychiatric Urgent Care offers telemedicine and "walk in" mental health urgent care (321) 586-5444.
For patients in need of support NOW, we often refer to Healthfirst Intensive Outpatient Programs and Palm Point Behavioral Health at (321) 603-6550 inpatient stabilization and intensive outpatient programs - both of which include therapy and medication management as part of their services, as well as La Amistad, which offers longer residential stays for mental health or substance abuse.
If symptoms are severe, call 911 or go to your nearest ER, or call 1 (800) 273-8255, NAMI 1 (800) 950-6264.
*** If you are dealing with an exacerbation of symptoms needing assistance before we are able to schedule an initial evaluation - or if the patient is under 6 years old, please refer to your insurance company, or search Psychology Today to find an appropriate provider who can see you sooner.***
